Possible documentation to attach depending on the type of insured event:

• Employer verification
• A copy of medical documentation
• Reporting workplace injury
• Reports on temporary incapacity for work
• Copy of vehicle registration certificate and driver's license (in the event of a traffic accident)
• All police documentation in the event of a police
• Death certificate
• Death certificate, autopsy report or death notice
• Certificate of succession including copies of identity cards and account numbers of the heirs
• Other

Final provisions

By submitting the claim the client gives his/her consent to P&T Rizik to use his/her personal data, as well as all other data that P&T Rizik comes in contact with when providing insurance brokerage services, for the purpose of resolving compensation claims addressed to the insurance company as well as use such data in its business records and process them in compliance with positive law and legal regulations.
